- Full name of the insurance company to which the application is submitted;
- Applicant’s full name, current place of residence or registration;
- A description of the applicant’s property claims for compensation for the damage and evidence supporting these claims;
- Data and information on the already conducted mutual settlements between citizens who have become participants in road accidents;
- The date of writing and submission of the application by the person, as well as the signature of the applicant.
In addition, the following package of documents should be submitted together with the application for compensation for damage caused in the event of an accident, namely:
- Documents certifying the applicant (for example, a passport when applying by an individual);
- Documents entitling the citizen to receive insurance payments from the organization (power of attorney, certificate of the right to inheritance) in cases where the applicant is not the injured party;
- RNOKPP – for individuals, EDRPOU – for legal entities;
- Documents confirming the death of the victim and his burial (required if the accident caused damage to the life and health of the person who died as a result);
- Information about the financial accounts of the applicant for further receipt of insurance payments.
