Obtaining the MAC Conclusion in form 080-4/o

In the Ukrainian healthcare system, the Medical Advisory Commission (MAC) plays an important role in confirming the health status of patients who require special treatment conditions, temporary disability or continuation of outpatient follow-up.  

One of the key documents issued by the MAC is the Conclusion in the form 080-4/o 

In practice, it is most often used not only in medical institutions, but also in the process of interaction of citizens with territorial recruitment and social support centers, in particular, to confirm long-term health disorders that affect the possibility of military service.  

At the same time, form 080-4/o is not a “permit” or a “deferment document”. It is only a medical report that can be taken into account by other authorities in accordance with the current legislation.  

Therefore, it is important to understand how to obtain this document correctly and legally, what medical grounds it must confirm, and how the entire procedure is built within the healthcare system.  

What is the Conclusion of the MAC in the form 080-4/o 

Form 080-4/o is an official medical document approved by the Ministry of Health. The MAC consists of several doctors of the relevant specialties, and its decisions are made collectively, which ensures objectivity and medical validity.  

The document is needed in different situations:  

  • for registration of disability,  
  • confirmation of chronic diseases,  
  • granting benefits,  
  • receiving social services,  
  • referral for sanatorium-resort treatment,  
  • as well as in cases where a person takes care of a sick relative who needs constant outside care 

That is why the form 080-4/o is often mentioned in the context of the procedure for obtaining a deferment from mobilization 

In cases where a citizen applies for a deferment on the basis of caring for a relative, the law requires confirmation that this person really needs constant outside help.  

This is why the MAC Conclusion is needed, because it officially certifies the presence of a disease, disorder or health condition that makes a person dependent on the care of another person.  

The MAC records that a person cannot independently provide for his/her basic needs, needs supervision or long-term treatment, and such a document becomes a key confirmation for state bodies that are considering granting a deferment for family reasons.  

Regulatory framework governing the activities of the MAC 

The work of the LCC is regulated by a number of orders of the Ministry of Health of Ukraine, in particular, documents that determine the procedure for drawing up forms of primary accounting documentation, including forms 080-4/o.  

The legislation does not provide for the paid nature of such certificates: they are issued free of charge, since they are part of the basic package of medical care.  

Grounds for applying to the MAC 

The grounds for issuing a conclusion of the MAC may be different.  

Among the main ones: 

  • chronic diseases that require regular monitoring or are at risk of exacerbation;  
  • conditions that limit physical activity;  
  • the need for long-term therapy;  
  • situations where the patient needs to obtain an opinion for other bodies or institutions.  

How to get the conclusion of the MAC: a step-by-step description of the procedure 

Initial visit to the family doctor  

Any medical procedure begins with a visit to a family doctor (sometimes to a specialized attending physician), it is he who forms the patient’s medical route, determines the necessary examinations and directs him to consultations with narrow specialists.   

The family doctor assesses the symptoms, analyzes the available medical documents and decides whether there are grounds for contacting the MAC. If there are grounds, the patient receives a referral or recommendations for collecting the necessary documents.  

Collection of medical documentation  

It is important for MAC to have a complete and up-to-date medical history. The patient must collect: test results, hospital discharges, conclusions of narrow-profile specialists, instrumental diagnostic data (ultrasound, MRI, CT, ECG, etc.).  

All these documents allow the commission to objectively assess the state of health and make an informed decision.  

Documents must be new or up-to-date. If a lot of time has passed, doctors may prescribe repeated examinations. The availability of up-to-date medical data not only facilitates the process, but also makes the conclusion more accurate.  

Passing specialists and diagnostics  

Depending on the nature of the disease, the patient may be referred to a neurologist, cardiologist, endocrinologist, orthopedist, gastroenterologist or other specialists. Each of them has the right to offer additional research.  

It is important to understand that MAC does not establish a diagnosis “from scratch”: it is based on already confirmed conclusions, so consultations and diagnostics are key stages.  

Submission of documents to the MAC  

After collecting the necessary information, the patient submits documents to the secretary of the MAC or through the electronic system of the institution.  

The commission sets the date of the meeting at which the case will be considered. This usually takes from several days to two weeks, depending on the workload of the medical institution.  

Consideration of the issue at the MAC meeting  

The MAC meeting takes place in the presence of several doctors who analyze medical data and make decisions. The patient may be present, but in some cases the examination is carried out without him, if all the documents are sufficiently informative.  

The commission may postpone consideration if additional examination or clarification of the diagnosis is required.  

Obtaining a conclusion in the form 080-4/o  

After making a decision, the secretary of the MAC draws up a document. The conclusion should indicate the diagnosis, functional limitations, recommendations for treatment and the timing of the re-examination.  

The document is signed by all members of the MAC and certified by a seal.  

Typical difficulties and nuances to pay attention to 

Sometimes patients are faced with the fact that the MAC refuses to issue a conclusion. The reasons can be different: insufficient medical documents, lack of a confirmed diagnosis, requirement to undergo additional procedures, or inaccuracies in medical records.

  • free receipt of the MAC conclusion in state medical institutions;  
  • familiarization with the materials on the basis of which the decision was made;  
  • appeal against the refusal to issue a document;  
  • reconsideration in case of a change in health status or the appearance of new medical data.  

Any requirements for “payment for the conclusion” are illegal, and the patient can apply to the administration of the institution or regulatory authorities.  

Practical advice 

To make the procedure for obtaining form 080-4/o faster and without problems, you should:
