Conclusion of the MAC on the need for permanent external care

The Medical Advisory Commission (MAC) is a collegial body in a medical institution that examines issues related to patients’ health and makes decisions regarding the need for medical measures, establishing disability, etc.

What does MAC do?

  1. Determines the need for permanent outside care.
  2. Gives conclusions for obtaining social benefits or benefits.
  3. Confirms or denies the need for medical rehabilitation or treatment.

The conclusion of the Medical Advisory Commission (MAC) on the need for permanent outside care plays a key role in resolving issues related to border crossing, deferral of conscription, and discharge from military service.

The main condition for issuing such a conclusion: the person must need constant external care, i.e. round-the-clock assistance of another person. This means that she cannot perform basic life functions on her own without help.

When which may need constant care (not an exhaustive list)?

  • Serious somatic diseases: oncological diseases in the terminal stage, severe cardiovascular diseases, chronic renal failure, etc.
  • Severe mental disorders: schizophrenia, bipolar disorder, severe depression, etc.
  • Severe neurological diseases: stroke with severe consequences, Alzheimer’s disease, dementia, etc.
  • Serious injuries: fractures with complications, burns, brain injuries, etc.

It is important to understand that the concept of “continuous care” is quite relative and may vary depending on the specific situation. For example, a person may need constant supervision, but be able to eat independently. Or, on the contrary, to be physically independent, but need constant psychological support.

The phrase “constant care” in the conclusion of the MAC is key in the context of deferment from conscription, crossing the border and exemption from military service.

How does the examination procedure at the MAC take place?

  1. Application submission. You contact the medical institution where you are registered and submit an application in the name of the head of the MAC.
  2. Collection of documents. Collect all necessary documents.
  3. Examination by specialist doctors. You are examined by doctors of various specialties, depending on your disease.
  4. Making a decision. The MAC considers all submitted documents and examination results and makes a decision.
  5. Obtaining a conclusion. After the decision is made, you will be issued a conclusion of the MAC.

What documents are required for MAC?

In order to apply to the medical advisory board, you need to prepare the following documents:

  1. Referral from a doctor — your family or attending physician draws up a document justifying the need to consider your issue.
  2. The patient’s medical card — with the history of diseases and the results of recent examinations.
  3. Results of analyzes and diagnostic studies — X-ray, ultrasound, MRI or other studies depending on the state of health.
  4. Documents confirming the patient’s identity (passport or other identification document).
  5. Other special references (if any), for example, disability documents or previous doctors’ opinions.

Do all people over 80 need constant care?

No, this is not true, although such a stereotype is quite common.

In the past, reaching the age of 80 in itself made it possible to receive public assistance for the care of an elderly person. Probably, this was the reason for the emergence of this myth.

To date, the mere fact of reaching 80 years of age does not provide grounds for receiving state assistance. The need for constant care depends solely on the state of health, not on age.

Therefore, if we are talking about the organization of permanent care for an elderly person, for example, a grandmother who is over 80 years old, an official document confirming the need for care (a conclusion of the Ministry of Health and Welfare or MAC) is required.
