When is a certificate of injury circumstances issued?
In cases where a serviceman has been injured during military service/defense of the Motherland, a certificate of wounds, contusions or mutilations is issued. It is prepared by the head of the medical service on the basis of an order from the military unit command. To prepare the latter, supporting documentation is required:
- signed report of the commander
- combat report
- information in the combat log
- primary medical record
- other materials certifying the fact of injury.
If the commander has not provided such data, the military man must independently formulate an explanation and send a report with a request to issue the document.
Why do you need a certificate of the circumstances of the injury?
This document is the basis for confirming the causal relationship between the injury and military service. It is needed when:
- passing the MMC and receiving disability;
- calculation of one-time financial assistance;
- payment of additional compensation and entering information into the personal file.
The certificate contains information such as the circumstances of the injury, the condition of the serviceman at the time of the incident (in particular, the absence of alcohol or drug intoxication), as well as information about possible violations.

Report on obtaining a certificate of injury circumstances

The report must be accompanied by an explanation from the serviceman, and if available, an explanation from two or three witnesses, as well as copies of medical documents (discharge summary, conclusion of the MMC, etc.). There are several ways to submit a report:
- in person at the military unit headquarters with document registration;
- through the military registration and enlistment office/medical institution, if the serviceman is undergoing treatment or outside the unit;
- by mail – registered letter with a description and notification of delivery.
It is important to keep a copy of the report and confirmation of its submission. If the commander refuses to issue a certificate, it is necessary to demand a written refusal and, if necessary, appeal it to the headquarters of the operational command or the Military Law Enforcement Service.
The maximum period for issuing a certificate in cases where the injury was caused by the actions of armed aggression of the russian federation is up to five days. If the circumstances require additional clarification, an official investigation is conducted.
What to do if you are not provided with a certificate about the circumstances of the injury?
There are often cases when a military man faces a refusal or delay in issuing a certificate. If the certificate is not provided, the following actions should be taken:
- file a complaint about the inaction of the military unit to the headquarters of the operational command;
- contact the Military Law Enforcement Service of the Armed Forces of Ukraine;
- call the hotline of the Ministry of Defense, recording the number of the request.
If there is no response, the applicant has the right to apply to the court with an administrative claim, which may recognize the decision as unlawful and oblige the issuance of the document.
