How to file a complaint with the Ministry of Health of Ukraine?

If your rights as a patient have been violated, you can contact the Ministry of Health of Ukraine with a complaint. There are several ways to do this:

The most convenient way is to fill out the official citizen appeal form. It must include:

  1. last name, first name and patronymic;
  2. mailing address for reply;
  3. email or phone number for feedback;
  4. the essence of the appeal with an indication of facts and circumstances.

After sending, your complaint will be officially registered in the Ministry of Health system.

  • Email

The complaint can be sent to the official address: ez@moz.gov.ua. The letter must include personal data, contacts and a detailed description of the problem. Letters without a full name or address may remain without consideration, as the appeal must meet the requirements of Article 5 of the Law of Ukraine "On Citizen Appeals".

  • Postal submission

You can also prepare a written complaint and send it by regular mail to the address of the Ministry of Health. This method is suitable for those who prefer official documents in paper form.

Important: regardless of the method chosen, the appeal must be clear, contain factual data (dates, names, circumstances) and specific requirements.

What should an appeal to the Ministry of Health contain?

In order for a complaint or application to the Ministry of Health to be accepted for consideration, it must meet the requirements of the Law of Ukraine "On Citizens' Appeals". The following data must be specified:
